Our team of veterinarians and nursing staff is specially trained and experienced in emergency medicine. No appointment is necessary, but please call ahead so we can prepare for your arrival. If you need assistance transporting your pet from your vehicle into the clinic, just let us know and we’ll be happy to help.

Client Intake Forms

Admission Form
We require this form for all new patients to gather essential information about your pet’s medical history, current condition, and contact details. Please fill it out upon arrival or submit it online in advance.
Estimate Authorization Form
By completing this authorization, you approve the estimated costs for treatment. It allows us to proceed with emergency care after your consent is given.
Anesthesia Release Form
Before your pet undergoes anesthesia, this release provides consent for the procedure and acknowledges the risks involved. Please review and sign prior to treatment.
CPR/DNR Form
This document allows you to specify your preferences regarding CPR and resuscitation in the event of an emergency. We will honor your decisions during critical care.
Third-Party Payment Authorization Form
If someone else is handling payment for your pet’s care, this authorization allows us to process the payment on their behalf. It ensures a smooth financial transaction during your visit

Paying for Your Pet’s Emergency Care
As is customary for emergency care, payment is due at the time of service. Aside from the examination fee, if diagnostic tests or treatment is recommended, you will be presented with a treatment plan and associated costs. We do our very best to provide you with all of the information you need to make a decision that is right for you and your pet. Please do not hesitate to ask questions if there is anything that you do not understand. We know this can be a stressful experience and want you to feel as though you are well equipped to do what is best.
Emergency hospitals typically have higher fees than general, day time practices due to the additional equipment, 24/7 overhead, and emergency training. If you are concerned about being able to manage costs in an emergency, we recommend you look into pet insurance before you need it. There are many great options available today with a variety of coverage levels. For your convenience, we accept Visa, Mastercard, Discover, and cash. We also have financing options available including CareCredit and Scratchpay for qualified applicants.
